Primary Contact Name iThe person who will be the main point of contact for onboarding and ongoing communication. State(s) of Operation iList every state where you operate or plan to operate. Medical director requirements vary by state. Number of Locations iTotal number of physical locations that will need medical director coverage. Each location is added to your agreement.

Primary Location iThe main location where the assigned physician will be listed as medical director. Other locations are covered under the same agreement.

2. Licensing & Staffing

Upload Business License iA copy of your current business license. Accepted formats: PDF, JPG, or PNG. ↓ Upload Esthetician License(s) iUpload licenses for all estheticians who will perform services under medical director oversight. 4. Timeline & Readiness

5. Malpractice Insurance

Upload Malpractice Policy (optional) iYour malpractice insurance policy. It must name the medical director as a covered party. You can upload this now or add it later through your onboarding link. ↓

Your email address will be your login for the client portal, where you can track your onboarding status, compliance progress, and upload documents.
